They also do NOT apply to the print bed, only to the model. Once those surfaces are defined though the support generation algorithm takes over. At the moment (as of 2.3.x and earlier) the generation is very aggressive.

And, remember this is a Prusa software set. What you and I think is 45 degrees, Prusa thinks is 90 degrees. So the range you can adjust in the setting is o (automatic) to 180 (perpendicular to the bed). Where 90 provides supports for a 45 degree angle the rest of us see.

Prusa "Organic Supports" are based on this Tree Support 2.0 Fork for Cura btw. (Prusa adds a bit on top to improve them even further) If you're interested and can't wait or are too annoyed by Cura's default Tree supports, you can try them out over there. The GitHub Page also explains the differences to Cura's stock Tree Supports quite well.Prusa's organic support are based on the new algorithm (by u/Thomas_Rahm), but they improve it a bit further by making the branches smoother and rounder for stability (which is what makes them "organic").
